By checking the boxes, I acknowledge and agree to the following terms related to my membership at The Woods Yoga, as outlined in my specific membership agreement:
Cancellation requests require 30 days notice. If my next payment is set to run during the minimum required period, or if it is past due, I am still obligated to make that payment, and changes will go into effect for my next billing cycle.
If I terminate my membership before completing my current term, I will be required to pay an early termination fee plus any outstanding membership payments, both of which are due at the time of cancellation.
Freeze requests require 48 hours notice. If my next payment is set to run during the minimum required period, or if it is past due, I am still obligated to make that payment, and changes will go into effect for my next billing cycle.
I am eligible to freeze my Membership once per calendar year for up to thirty (30) days at no charge. Additional freezes may be allowed for a fee, for up to two (2) months, for a total of three (3) months of freeze time per year. During a freeze, memberships cannot be canceled. If a cancellation is requested while the membership is frozen, the 30-day notice period will begin on the date the membership becomes active again. The freeze period does not count toward any minimum commitment period.
Supporting documentation is required for relocations and extended medical leave requests. Please submit with your request.
